Make it fit the send.

• The answers are a list of label and link, at least two; each link records its own answer, so a three-point scale and a five-point scale come from the same source. Point each link at your own endpoint.
• The scale's end labels are optional, for a numbered scale whose numbers need words; long-content shows them. Name the sender and their role, and send from someone who can act on the answer.
• The incentive line is optional: omit it, as the default fixture does. There is no picture on purpose, so it reads as a note from a person. It is written for Clementine, a fictional booking app; replace its mark and restyle through starter.css.
